Tax Forms & Resources
The Library offers several resources to assist you with tax preparation including tax forms, tax preparation websites, and tax preparation books. Library staff cannot help you fill out tax forms or recommend specific forms to use.

A large selection of State of Ohio and City of Centerville tax forms and instructions is available for free pickup at either Centerville or Woodbourne Library. The selection of Federal forms available for free is limited to those provided by the IRS.

Can't find a particular form you need? See a Reference Librarian for assistance. We can retrieve most forms from the IRS website. We also stock a variety of tax handbooks with useful tax information from the industry leaders.

Some forms cannot be printed from the IRS website, including 1099 and W-2 forms. Copies of these forms must be ordered directly from the IRS by calling 1-800-TAX-FORM (1-800-829-3676) or by contacting our local IRS Office at 200 W. Second Street, Dayton, OH, 45402, (937) 610-2182.

You can also get information about how to order federal forms and publications by mail on the IRS website.
