Consent Form:

Yes, I would like to be notified of my holds (reserves), overdues, and upcoming due dates via email instead of by mail.

Individual titles will be listed on notices. I understand and accept that this information will be visible to all who have access to the email account.

Holds:
Only one hold notice is sent when the item(s) is available.
You have 10 days from the notice date to pick up the item(s). After 10 days, the item(s) will go to the next person on the hold list or be re-shelved. Video games must be picked up within 5 days.

Overdues:
The first and second overdue notice are sent via email. All billing notices are sent via regular mail.

Upcoming Due Dates:
Reminder notices of upcoming due dates are sent three (3) days prior to the due date for items with loan periods longer than one week. The Express Notice service is not offered for items with loan periods of one week or less.
