Passport Services
Passport processing is now available by appointment at both the Centerville and Woodbourne Library.

Centerville Library Passport Hours:
  • Monday & Wednesday, 6-8 PM
  • Saturday, 10 - 3 PM
Woodbourne Library Passport Hours:
  • Tuesday & Thursday, 6-8 PM
  • Saturday, 10 - 3 PM

You will need to bring the following with you to your appointment:
  • Proof of U.S. citizenship (one of the following:)
    • Previously issued, undamaged U.S. passport
    • Certified birth certificate issued by city, county, or state
    • Naturalization certificate
  • Current, valid driver's license, government ID or military ID (Note: A Social Security card is not a valid form of identification.)
  • 2 (two) identical passport photos, which must be:
    • 2" x 2" in size
    • taken within the past 6 months, showing current appearance
    • in color
    • full face, front view with a plain white or off-white background
    • between 1 inch and 1 3/8 inches from bottom of chin to top of head
    • taken in normal street attire

  • Passport Book Fee ($110 Adult; $80 Minors) AND/OR Passport Card Fee ($30 Adult; $15 Minor) payable to "U.S. Department of State" (check or money order only)
  • Execution Fee ($25) payable to "Washington-Centerville Public Library" (Cash, check, Visa, or Mastercard)

Notary Services
Free notary services* are available at Woodbourne Library (937-435-3700) and Centerville Library (937-433-8091), Monday through Saturday. Please call in advance to verify notary availability.

*The Library's free notary service is intended for simple documents (auto titles, etc.) that can be notarized in 30 minutes or less and do not require specialized expertise. Library staff may reserve the right to refuse a request that exceeds the intention of the service and/or prevents staff from fulfilling the primary mission and goals of the Library. To find assistance with more complex documents, websites such as 123notary.com can help locate notaries in your area.

In order to best serve you, please note the following notary guidelines:
  • Please bring a valid government issued photo ID and unsigned documents to be notarized. Documents must be signed in the presence of the notary in order to be valid.
  • Please complete all information above the signature line completely.
  • Notary service ends 30 minutes prior to closing.
  • Patrons must bring their own witnesses, if needed.
  • Please limit notary requests to no more than 5 documents per patron, per day.
Voter Registration
To register to vote at Woodbourne or Centerville Library, visit the front desk and ask for a registration form. You can check to see if you are registered to vote, determine your voting place, download registration forms, and register to vote online at http://www.sos.state.oh.us/voter.aspx. In order to vote in an upcoming election, you must be registered thirty (30) days prior to the election.
